- This LOA must be approved, otherwise I will be considered absent.
- My request cannot be backdated unless there are mitigating circumstances such as hospitalization which prevented the request to be made sooner (proof of such circumstances must be obtained).
- The maximum allowable LOA is 7 consecutive days within a 30-day period unless the leave is for a medical reason and you have a doctor's note specifying more time is needed.
- I am allowed to take multiple leaves of absence.
- The total of all of my leave of absences may not exceed 180 days in any 12-month period.
- I will not incur any additional tuition charges during any LOA.
- I must return on the date stated above or I will be considered an unofficial drop.
- In the event that I do not return as scheduled, any refund due will be made to the appropriate financial aid programs within 45 days* of the date I was scheduled to return.
- Any request for an extension of LOA must be submitted by 11:59 PM of the day you were scheduled to return.
Acceptable reasons for a leave of absence include a temporary situation beyond the student's control, such as personal or family illness, death of an immediate family member, military or jury duty, conflicts in work schedules, transportation problems, financial issues and pre-approved travel or commitment.
Students taking an LOA for medical reasons must submit a doctor's excuse as soon as possible, but at least within 7 days of the start of LOA for approval and a doctor's release must be submitted to Student Records before returning to training.
